- Valid Referral from your GP or Specialist
- Medicare Card
- Private Health Insurance Card (if applicable)
- Relevant Test results
- Relevant X-rays, CT scans, MRI scans
- Method of Payment (Cash, EFTPOS, Mastercard/Visa)
